I charge 400 dollars for each visit with sliding scale. I usually see patients about every 1-2 months.

CANCELLATION POLICY

I request at least 24 hours notice for cancelled appointments. I do charge for missed visits.

INSURANCE

I am an OUT OF NETWORK PROVIDER. I am happy to provide a SUPERBILL for insurance.

SCHEDULE

I work on TUESDAYS and THURSDAYS from 7 AM to 2 PM and 3 PM to 5 PM.

AVAILABILITY

I am usually booked out about 1-2 weeks, but will try to squeeze you in.

COMMUNICATION

I am always available by TEXT or EMAIL and usually respond within 24 HRS.

PAPERWORK

I am comfortable completing necessary paperwork and rarely charge for it.
